California’s Taxpayer Dollars Spent on Gender-Related Surgeries for Inmates
California has allocated over $4 million of taxpayer money to fund gender-related surgeries for inmates, including those on death row, since 2017.
The Washington Free Beacon released a report revealing that millions of dollars have been spent on sex changes and other gender-related procedures for 157 inmates in California. This makes California the first state to use taxpayer funds for such operations, with four of the recipients being death row inmates.
The report states that since 2017, California has paid for 35 male convicts to undergo surgeries for artificial vaginas and vulvas, costing taxpayers $2.5 million. Additionally, 11 male prisoners received breast implants, amounting to over $180,000 in expenses. The state government also covered the cost of breast removal surgeries for 40 female detainees, totaling more than $1 million.
Harmeet Dhillon (R-Calif.), National Committee Chairwoman, expressed her belief that while transgender individuals deserve respect, using taxpayer money for experimental and scientifically unjustified surgeries is not justified.
In 2023 alone, California prison authorities requested an additional $2.2 million for transgender operations. This budget included the hiring of two full-time psychiatrists, one physician, one surgeon, and two social workers.
California became the first state to require taxpayer funding for transgender inmates’ surgeries in 2017, following a legal settlement with Shiloh Quine, a biological male prisoner who sued the state for recognition as a transgender woman and access to specific treatments and procedures. The number of transgender inmates in California has significantly increased since then, with 1,847 individuals identifying as transgender or “non-binary” as of November 2023.

Should the cost and medical necessity of gender-affirming surgeries for inmates be further evaluated to ensure responsible use of taxpayer funds
California’s Taxpayer Dollars Spent on Gender-Related Surgeries for Inmates
California has attracted attention and sparked controversy by allocating over $4 million of taxpayer money to fund gender-related surgeries for inmates, including those on death row, since 2017. The recent report by The Washington Free Beacon shed light on this matter, revealing that California is the first state to use taxpayer funds for such operations, with four death row inmates among the recipients.
According to the report, since 2017, California has financed surgeries for artificial vaginas and vulvas for 35 male convicts, costing taxpayers $2.5 million. Additionally, 11 male prisoners received breast implants, amounting to over $180,000 in expenses. The state government also covered the cost of breast removal surgeries for 40 female detainees, totaling more than $1 million.
This news has garnered mixed reactions from lawmakers and the public. Harmeet Dhillon (R-Calif.), National Committee Chairwoman, expressed her belief that while transgender individuals deserve respect, using taxpayer money for experimental and scientifically unjustified surgeries is not justified. This raises questions about the ethical and financial implications of using taxpayer funds for such procedures.
Furthermore, California prison authorities requested an additional $2.2 million for transgender operations in 2023 alone. This budget included the hiring of two full-time psychiatrists, one physician, one surgeon, and two social workers. It demonstrates the increasing demand for gender-related surgeries within the inmate population and the subsequent strain on the state’s resources.
California’s decision to require taxpayer funding for transgender inmates’ surgeries dates back to 2017, following a legal settlement with Shiloh Quine, a biological male prisoner who sued the state for recognition as a transgender woman and access to specific treatments and procedures. Since then, the number of transgender inmates in California has significantly increased, with 1,847 individuals identifying as transgender or ”non-binary” as of November 2023.
While the intention behind providing gender-affirming surgeries is to promote inclusivity and address the healthcare needs of transgender inmates, the use of taxpayer funds for these procedures remains a contentious issue. Critics argue that the cost and medical necessity of some of these surgeries should be further evaluated to ensure taxpayer money is being spent responsibly.
As California continues to lead the way in this area, it is likely that the debate surrounding taxpayer funding for gender-related surgeries in prisons will persist. Striking a balance between providing appropriate healthcare and responsibly managing taxpayer funds will be crucial in shaping future policies and practices in this domain.
In conclusion, California’s allocation of taxpayer money for gender-related surgeries for inmates has sparked debate and raised questions about the ethical and financial considerations involved. With the increasing demand and associated costs, finding a middle ground that prioritizes healthcare needs while ensuring responsible use of taxpayer funds will be crucial moving forward.
